DEPARTMENT: HIV Prevention Program
JOB TITLE: PrEP Navigator
REPORT TO: HIV Program Manager
BASIC FUNCTION: Under the director supervision of the Prevention Services Supervisor, the PrEP Navigator is responsible for assisting in the promotion and implementation of HIV Prevention Services.
REPRESENTATIVE DUTIES: The duties recorded below are intended to accurately represent the duties of the job and are not intended to cover every single duty of the job.
1. Focus services to link HIV positive individuals into care and offer anti-retroviral therapy (ART)
2. Focus services to recruit and offer Pre Exposure Prophylaxis (PrEP) and PrEP Navigation to HIV negative results
3. Provide PrEP Navigation services to HIV-negative patients including appointment making, enrollment in Patient Assistance Programs (including PrEP-AP), treatment adherence education, and follow- up support services
4. Complete PrEP-AP training to become a PrEP-AP enrollment worker and provide PrEP-AP services to qualifying HIV-negative clients
5. Work with program staff and supervisor to plan program components (i.e. outreach in-person and online, social marketing, recruitment and clinic based services).
6. Offer and document Partner Services to HIV-positive clients with a detectable viral load.
7. Promote, offer and track HIV and HCV testing for those people unaware of their HIV status engaged in high –risk behaviors
8. Collect and enter process and outcome data for services in NextGen databases. Prepare monthly accurate narrative and statistical program reports
9. Participate in health resource fairs and community events as needed. This includes maintaining a flexible schedule to allow for some after-hour/weekend work as well as emergency coverage of shifts to meet the needs/demands of clients and service related activities and events.
10. Support and actively participate in all internal and external meetings and staff development trainings. Represent Clinicas de Salud del Pueblo, Inc. and HIV Department
11. Perform other duties as assigned
EDUCATION AND EXPERIENCE: High school diploma or GED equivalent. HIV Test Counselor, Partner Services training or any other HIV prevention training preferred. Combination of education and years of experience as previously specified. Sensitivity to LGBT-related cultural concerns and the prevention certifications and training concomitant to HIV disease. Previous HIV field experience a plus. Experience in group facilitation and educational instruction.
KNOWLEDGE AND ABILITIES: knowledge of prevention strategies sand interventions. Demonstrate ability to be culturally sensitive and appropriate in working with communities of color. Demonstrate respect for diversity. Knowledge of HIV/AIDS and its physical, psychological & emotional effects on individuals and family units. Knowledge of health and social issues people with HIV/AIDS. Exercise tact, objectivity, sensitivity, strategy and judgment in dealing with a variety of people with a variety of co-occurring disorders. Organize, set priorities and exercise sound independent judgment within areas of responsibility. Establish and maintain effective working relationships with clients, family members, other professional staff and the public. Strong customer service and organization skills, excellent oral and written communications skills. Bilingual in English/Spanish preferred.
LICENSE/CERTIFICATIONS: Access to an insured vehicle and a valid Class C California driver’s license. HIV Test Counselor and Partner Services training within six months of employment.
WORK DIRECTION, LEAD AND SUPERVISORY RESPONSIBILITIES: Not applicable-no assigned permanent staff to supervise.
PHYSICAL EFFORT: Sitting, standing, and walking for extended periods of time. Lifting 25-40 lbs. Continuous contact with clients. Travel to any satellite facility. May work evenings, nights, and/or weekends.
