We are seeking a full-time Intake Coordinator to support our growing team. This is an in-person position based at our Monterey office. In this role, you will be at the center of our operations—managing patient referrals, coordinating intake processes, and ensuring a seamless experience for patients and families.
What We’re Looking For
- Knowledge of Medicare guidelines, insurance verification, and state regulations
- Strong communication and customer service skills
- Ability to manage high-volume data entry with accuracy and efficiency
- Excellent organizational and multitasking abilities
- Experience in home health or healthcare intake preferred
Key Responsibilities
- Process incoming referrals and service requests in a timely manner
- Complete and verify intake documentation, including medical and demographic information
- Communicate with patients, families, referral sources, and clinical staff
- Verify insurance eligibility and benefits
- Maintain accurate logs, records, and compliance documentation
- Collaborate with clinical, billing, and operations teams to ensure smooth admissions
- Provide a high level of customer service and support throughout the intake process
- Ensure all patient information is handled with strict confidentiality and compliance

Qualifications
- High school diploma or equivalent (preferred)
- Minimum 1 year of intake or healthcare coordination experience (preferred)
