Your Impact

The Social Services Coordinator has the primary responsibility of securing/supporting/coordinating behavioral health services and psychosocial support that encourage and support an environment which is least restrictive to the psychosocial health to a resident of Inglis House. The Social Services Coordinator should provide concrete services, evaluate mood, monitor behavior, and implement appropriate interventions, as evidence by developing behavioral plans when needed which may include working with resident families and their concerns. The Social Services Coordinator will serve as an advocate for the residents as well as a liaison to the facility, family members, and representatives from other entities. All essential job responsibilities will be executed in accordance with Inglis Administrative procedures and compliance/ethics guidelines.

What You'll Do

Essential Functions

Concrete Services to Residents

  • Ensure transportation applications are filled out for residents within the indicated deadline.
  • Responsible for being assessable and responsive to residents’ and/or family’s requests and ensuring phone calls are promptly returned; within 24-48 business hours.
  • Coordinates and facilitates concern process refers concerns to appropriate staff for resolution.
  • Supporting grievance investigations and communication of the resolution to grievances to residents and/or their representatives as needed within the timeframe specified by Inglis House policy. Assist with discharge planning as needed and requested by the Director of Social Services.
  • Assists with additional tasks and responsibilities as deemed necessary by Director of Social Services and Nursing Home Administration.

Advocating for residents of Inglis House/Residents Rights

  • Develops a comprehensive social history and psychosocial assessment that includes the resident areas of need, strengths, and preferences, including cultural, religious, ethnic, and mental health background and implications for the care plan.
  • Provides a consistent forum for helping residents express their needs, concerns, and wishes.
  • Involves residents, families, and designated representatives in care planning and encouraging their attendance at care conferences.
  • Attends scheduled care conferences and identifies and interprets psychosocial needs of residents for inclusion in the care plan, addressing issues of schedule, treatment and environment that express and reinforce individuality and identity.
  • Completes mandatory documentation within timelines established by applicable regulations.
  • Will support residents during meetings and discussions to ensure resident’s needs and preferences are considered.
  • Inform residents of Ombudsman services when needed.
  • Schedules and facilitate family meetings as needed.
  • Informs residents and families of services to assist with completion of Advanced Directives, Living Wills, Power of Attorney Paperwork etc. during admission process and when needed and supports as needed.
  • Reports and addresses possible allegations of abuse and/or neglect as they occur.
  • Interviews affected residents and other residents as determined when allegations are made and assisted with securing identified areas of support.
  • Documents findings of reported allegation and reports to Nursing Home Administrator within 24 hours of findings.
  • Reviews and documents of resident rights with resident on an annual basis.

Documentation

  • Progress notes for behavioral concerns are completed and charted within one (1) business day of resident contact.
  • MDS are completed within five (5) business days of the assessment reference date.
  • Social Work assessments are completed and charted within fourteen (14) days of admission.
  • Completes quarterly care conference documentation within one business day.
  • Living Wills and Advanced Directives are reviewed for accuracy during care conferences and as requested.
  • Quarterly & Annual notes are completed and reviewed for accuracy within the specified time. frame as determined by the supervisor.
  • Updates, revises, and adjusts residents’ care plans based on MDS assessments, witnessed and reported behaviors, psychotropic medications, discharge disposition, and identified and discussed psychosocial and behavioral health supports. Care plans will also address person-centered care approaches, and interventions attempted to meet the residents’ needs.

Behavioral & Mental Health Interventions

  • Complete quarterly assessments and quarterly notes.
  • Attends quarterly Gradual Dose Reduction Meetings.
  • Consults with Director of Social Work, counselors, and Psychiatrist to develop behavioral plans and contracts as needed.
  • Consults with interdisciplinary team, Nursing Home Administrator, resident, and support systems to develop behavioral/conduct agreements as needed.
  • Consults with residents, support systems, and interdisciplinary teams to develop and implement person-centered interventions to residents of Inglis House.

What We're Looking For

Education and Work Experience

Minimum of a bachelor’s degree in social work or a bachelor’s degree in a human services field (required) including, but not limited to, sociology, gerontology, special education, rehabilitation counseling, and psychology.

One year of supervised social work experience in a health care setting working directly with individuals.

Must be able to function independently and have flexibility, personal integrity and the ability to work effectively with residents, staff, and support agencies.

Must be able to relate to and work with ill, disabled, elderly, emotionally upset and at times hostile people within the facility.

Must possess the ability to make independent decisions when circumstances warrant such action. Ability to make psychosocial assessments and develop and implement viable care plans.

Excellent listening and communication skills.

Pay Rate for this Role: $55,227-$72,220/Annually
