Required Skills and Qualifications:
- Has obtained MA RN or LPN license.
- Having a previous work experience in the Geriatric field or a Skilled Nursing Facility is preferred.
- Must have the ability to communicate effectively, including written and verbal skills; work well, both individually and as part of a team, through respect, collaboration, and dedication; and successfully interact with a diverse culture base.
- The successful candidate must prioritize and organize work effectively and efficiently and demonstrate a positive attitude, high integrity, and ethical awareness.
Duties and Responsibilities:
- Schedule doctor appointments, prepare charts, and assist physicians during rounds.
- Manage new admissions and hospital returns; update and maintain care plans.
- Monitor residents’ weight and nutrition; report and follow up on risks with Dietary.
- Orient and assign nursing staff to meet unit needs.
- Observe and record resident conditions; report changes and update charts.
- Develop and provide resident and family education as needed.
- Supervise and assist with daily living activities.
- Coordinate nursing services with other departments and perform other assigned duties.
