Job Title: FNP
Functional Group: NP
Duration: 6 months (Extendable or contract to hire)
Schedule: this position will require working every other weekend with possible holiday rotations M-F 830-530P & 40 hours in a week (OT is also available).
Requirements:
Master's Degree level Family Nurse Practitioner program with current National Board Certification and State of Employment license to practice in the Advanced Practice Nurse role. A minimum of 1 year of recent Primary Care experience as a Nurse Practitioner in either a family medicine or primary care practice preferred
Duties may include but are not necessarily limited to the following:
Nurse Practitioners will work in collaboration with a dedicated clinical care team to provide evidenced-based care to a panel of patients. The clinical care team will work with the provider daily to drive better outcomes for entire panel of patients. This will include accurate assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning. This role will report to the practice manager.
