Care Coordinator

Job Summary:

Key Responsibilities:

• Conduct interviews with patients or their designees to gather relevant information and assess the effect of illness on the patient and their family.
• Participate in the development of interdisciplinary care plans and coordinate discharge planning processes.
• Provide therapeutic support and education to patients and families, ensuring they understand medical recommendations and have access to necessary resources.
• Document results of assessments and plans in patient medical records, meeting regulatory compliance and departmental standards.

Requirements:

• Bachelor's degree in Social Work (BSW) required; Master's degree in Social Work (MSW) preferred.
• Valid State of Alaska driver's license.
• Minimum 2-4 years of experience in inpatient or outpatient healthcare environments in social work.
• Demonstrated understanding of sound ethical practice and knowledge of community support resources.
