Responsibilities:
The Spiritual/Bereavement Coordinator works in conjunction with the Interdisciplinary Group to:
- Make contact with the patient/family or other clergy members in accordance with the hospice plan of care
- Function as a liaison between the IDG and the religious and lay community
- Provide direct spiritual/bereavement support to patients/families as requested
- Responsible for the documentation of actions into the patient’s medical records
- Responsible for assuring completion of Spiritual Assessment within 5 calendar days of patient’s admission
- Responsible for assuring completion of the Pre-bereavement Assessment within 5 calendar days of patient’s admission
- Supervise the Bereavement program, discharging those patients who no longer desire our services
- Collaborate with other agencies and professional groups when needed to meet the needs of grieving family members
- Records appropriate documentation of actions in patient/family records
- Completes reports of projects and activities as directed by the Director of Counseling or designee
- Facilitates community education topics as directed
- Participate in IDG meetings, case conferences
- Participate in CQI activities as requested
- Participate in after hours rotation as requested
- Other duties as assigned
Qualifications
Evidence of Chaplaincy training with at least 2 years of Pastoral experience. Demonstrated ability to counsel persons who may or may not be of the same religious background. Experience in counseling persons who have or are experiencing the grief and/or loss of a loved one. Ability to communicate effectively with patients/survivors under stress. This position reports directly to the Director of Counseling or Director of Patient Care Services in absence of Director of Counseling.
