Job Title: RN Hospice Case Manager

Location: Barre, VT 05641

Shift per week: Monday - Friday, 36hrs/week

Length of Assignment: 20 weeks (possibility of extension)


Required:

  • Experience with hospice case management
  • Experience providing hospice care in a home setting
  • Prior experience with rural travel preferred
  • Experience with McKesson/Netsmart EMR preferred
