Individual Support Services provides supported living and in home care to elderly, physically disabled and intellectually disabled clients. As part of our mission, we are looking for dedicated employees to fill the position of "Program Coordinator". As a Program Coordinator, you will be responsible for managing a caseload of clients to ensure the best quality of care. Managing a caseload would include establishing and maintaining relations with staff and clients, attending quarterly and annual meetings, training staff, maintaining accurate client records and information, completing required monthly reports, data tracking/analysis, serving as needed as a back-up direct care staff for caseload and ensuring that quality services are being provided.
A degree in social services or related work experience is preferred but not required. Ultimately, the position will be awarded to applicants who have a strong passion for helping others and are self-motivated to improve on the qualities necessary to succeed in this field.
This position is based at our Middletown office, but does require the ability to travel to surrounding areas in which services are provided. Position allows the option of a company vehicle. Main hours are 8am-5pm Monday through Friday, but can vary based on need.
