Duties
- Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility support
- Monitor patient health by observing vital signs and recognizing changes in condition; accurately document findings using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems
- Administer medications and perform basic medical procedures like catheter care and wound management following medical terminology guidelines and HIPAA regulations
- Support patients with memory care needs, including Alzheimer’s disease and dementia, ensuring a safe environment through specialized techniques in memory care and hospice settings
- Operate medical equipment such as Hoyer lifts for safe patient transfers; assist with mobility using appropriate techniques and safety protocols
- Prepare nutritious meals tailored to dietary restrictions; assist with feeding when necessary while maintaining patient dignity
- Perform light housekeeping duties including cleaning, laundry, and maintaining a safe living environment to promote overall health and comfort
- Maintain detailed medical documentation of patient care activities, vital signs, medication administration, and other relevant records in compliance with healthcare standards
Requirements
- Valid certification as a Home Health Aide (HHA) or Certified Nursing Assistant (CNA) with current state licensing where applicable
- Proven experience in home health care, long-term care facilities, nursing homes, or assisted living environments
- Strong knowledge of medical terminology, medication administration, and patient observation techniques
- Experience working with individuals with disabilities or developmental disabilities; familiarity with Alzheimer’s care and dementia-specific strategies is highly desirable
- Ability to operate EMR/EHR systems efficiently for accurate documentation of patient information
- Valid Work Authorization/Valid Photo ID/Signed Social Security card
- Pre Employment Physical, Drug screen, QuantiFERON, Rubella and Rubeola (Within the Past Year)
- Excellent communication skills and compassionate approach to patient care; ability to work independently and as part of a team
- Physical stamina to perform caregiving duties including lifting, transferring patients using Hoyer lifts, and standing for extended periods
