Job Title: Clinical Manager
Reports To: Director of Nursing (DON)
Location: Home Health Agency, Florida
Position Summary
The Clinical Manager supports the Director of Nursing in overseeing the day-to-day clinical operations of the agency. This role helps ensure that patient care is delivered safely, timely, and in compliance with physician orders, agency policy, Medicare/Medicaid requirements, accreditation standards, and applicable Florida regulations. The Clinical Manager provides direct supervision to clinical staff and serves as a key link between field clinicians, intake, scheduling, QA, and leadership.
Key Responsibilities
- Supervise and support nurses and other clinical staff in the delivery of home health services.
- Assist the DON in managing daily clinical operations, workflow, and staff performance.
- Review assessments, plans of care, recertifications, resumptions of care, discharges, and other clinical documentation for accuracy, timeliness, and compliance.
- Monitor adherence to physician orders, care plans, agency protocols, and regulatory requirements.
- Provide clinical guidance and problem-solving support to field staff on patient care issues.
- Help oversee case assignment, care coordination, visit utilization, and clinical scheduling efficiency.
- Participate in QA/QAPI activities, chart audits, incident review, infection control oversight, and corrective action planning.
- Assist with orientation, training, mentoring, and competency evaluation of clinical personnel.
- Support survey readiness and assist in responding to audits, ADRs, and regulatory reviews.
- Escalate significant clinical, compliance, staffing, or patient safety concerns to the DON promptly.
- Help ensure timely follow-up on hospitalizations, missed visits, patient complaints, and clinical variances.
- Collaborate with intake, billing, HR, and operations to support clean admissions, compliant care delivery, and strong clinical outcomes.
- Active Florida RN license in good standing required.
- Minimum of 2 years of home health clinical experience required.
- Prior supervisory or leadership experience in a certified home health agency strongly preferred.
- Strong knowledge of home health Conditions of Participation, documentation standards, and clinical compliance requirements.
- Strong organizational skills, clinical judgment, and ability to manage multiple priorities.
- Proficient in EMR/documentation systems and standard office technology.
