We are seeking to hire two experienced Registered Nurses (RNs) to join our team in a community-based, hybrid role supporting care management for older adults. These positions are centered on thoughtful coordination, clinical insight, and client advocacy. It is not hands-on caregiving, but focuses on ensuring each client receives aligned, high-quality, and well-coordinated care. We provide concierge-level care and service. We pay a high hourly rate and provide a higher level of work-life flexibility compared to our competitors.

• North: Downtown Houston to North Houston (1 RN for this region)
• South: Downtown Houston to South Houston (1 RN for this region)

Role Description
- This is a contract hybrid role for a RN Care Manager / Part Time RN / Per Diem RN, based in Houston, TX, with opportunities for remote work. The RN will provide care management services to older adults and their families, including assessing client needs, developing and monitoring personalized care plans, coordinating with healthcare providers, managing care transitions, and ensuring continuity of care. The RN will act as a liaison between clients, families, and medical professionals to advocate for the best possible outcomes.

A Day in This Role:
• Visit clients in their home or facility to assess medical, functional, and psychosocial needs
• Provide clinical support and oversight within individualized care plans
• Support medication management and ongoing wellness monitoring
• Transport and accompany clients to appointments, offering advocacy and coordination of follow-up care
• Collaborate closely with families and care teams to ensure care is appropriate, consistent, and supportive
• Maintain clear, ongoing communication with clients, families, and the administrative team
Qualifications:
- Proficiency in Nursing, Patient Care, and providing compassionate support to aging populations
- Strong background in Nursing Management and Critical Care Medicine
- Knowledge of Medicine, including understanding diagnoses, treatments, and care plans relevant to older adults
- Exceptional organizational, problem-solving, and communication skills
- Current Registered Nurse (RN) license in the state of Texas
- Experience in aging care, gerontology, or related fields is a strong advantage
- Comfort with a hybrid work model that includes in-person and remote responsibilities
- Passion for improving quality of life for older adults and their families
What We’re Looking For:
• Active RN license (Texas)
• Experience in home health, home care, long-term care, acute care, or rehab required
• Caregiving experience strongly preferred
• Dementia care experience preferred
• Hospice experience preferred
• Strong clinical judgment with a care coordination mindset
• Excellent communication and organizational skills
• Ability to work independently while staying connected to a team
• Valid driver’s license and reliable transportation, and willingness to drive to/with clients on a routine basis (we will discuss this during the interview process)
