Overview

Job Title: Social Worker – MSW / LCSW

Schedule: Saturday, Sunday, Monday, Tuesday (10-hour shifts)
Location: West Hospital, Avon, IN

Position Overview:

The Social Worker MSW / LCSW provides face-to-face psychosocial assessments, therapeutic interventions, and discharge planning support for patients and families. This role involves collaborating with the multidisciplinary healthcare team to ensure safe, timely, and effective care while addressing complex psychosocial and mental health needs. The social worker also serves as a key resource in abuse/neglect cases, guardianship, foster care, and mental health placements, providing crisis intervention and leadership in acute situations.

Key Responsibilities:

  • Meet face-to-face with patients and families to assess psychosocial needs and provide counseling.

  • Assist in developing educational materials for team members, patients, and families.

  • Serve as a resource and role model for students, interns, LBSWs, and other social work staff.

  • Participate with the clinical team to develop treatment plans aimed at resolving psychosocial issues and meeting patient needs.

  • Act as a consultant for providers regarding mental health diagnoses.

  • Effectively communicate and collaborate with all members of the healthcare team to facilitate safe and timely care.

  • Interpret and communicate complex patient and family needs; role-model caring practices.

  • Assess psychosocial risk factors, including prior functioning, support systems, reactions to illness, and coping mechanisms.

  • Counsel patients and families on emotional, social, and financial impacts of illness/disability.

  • Access and mobilize community and family resources to meet identified needs.

  • Conduct mental health assessments upon physician referral.

  • Facilitate complex discharge planning and therapeutic interventions.

  • Serve as the primary contact in cases of child abuse/neglect, elder abuse/neglect, institutional abuse, and domestic violence.

  • Act as liaison with child and adult protective teams (APS/DCS).

  • Maintain principles of crisis intervention and de-escalation, supporting safety in volatile situations.

  • Provide intervention support in guardianship, foster care, adoption, and mental health Level II placements.

  • Deliver specialized mental health services, including crisis intervention and addiction assessments, with recommendations for psychiatric referral or community resources.

  • Support individuals in acute crises and lead de-escalation efforts to prevent violence.

  • Utilize effective interviewing principles with victims, perpetrators, and significant others.

  • Provide intervention related to end-of-life, guardianship, foster care, and adoption cases.

  • Develop and maintain strong working relationships with hospital leadership, law enforcement, community agencies, schools, probation, and other stakeholders.

  • Respond to dangerous and potentially violent situations, including on-call and holiday rotations.

Qualifications & Requirements:

  • Education: Master’s Degree in Social Work (MSW) from an accredited program.

  • Licensure: Permanent Indiana LCSW required.

  • Certifications:

    • Certification in Crisis Prevention Intervention or equivalent de-escalation training required within 6 months of hire.

  • Experience: Relevant healthcare, mental health, or community-based experience preferred.

  • Knowledge & Skills:

    • Evidence-based psychosocial assessment, short-term therapeutic counseling, and crisis intervention.

    • Knowledge of disease processes and human behavior in relation to health-related social needs.

    • Complex discharge planning skills for safe patient transition.

    • Legal knowledge related to child and adult protective services mandates.

    • Principles of growth and development across the lifespan, including end-of-life care.

    • Ability to identify decision-makers and facilitate goals of care conversations.

    • Experience serving geographically, culturally, and financially diverse populations.

    • Strong interpersonal, communication, and collaboration skills.

    • Computer literacy and proficiency in electronic medical record documentation.

Additional Information:

  • Position includes working 4 days per week, with 10-hour shifts on Saturday, Sunday, Monday, and Tuesday.

  • May require on-call and holiday coverage.
