Indiana Masonic Home Compass Park is looking to add a Physical Therapist for Home Health. Compass Park is a vast 320-acre community in the heart of Johnson County adjacent to Indianapolis. It is a vibrant community through all seasons and a place of beauty, rich in tall trees and winding footpaths. This job is in Bloomington/Bedford area.
Position Summary
The Home Health Physical Therapist (PT) provides skilled physical therapy services to patients in their place of residence on an intermittent basis. This role aligns with physician orders, the individualized plan of care, and guidance from the Director of Clinical Services/Clinical Manager. The PT focuses on enhancing patients' mobility, strength, and overall physical function, ensuring their safety and independence within the home setting.
Key Responsibilities
- Patient Care and Therapy Delivery
- Conduct initial evaluations to assess patient needs, physical capabilities, and home environment.
- Develop and implement individualized treatment plans with specific goals tailored to each patient’s unique physical therapy needs.
- Perform skilled therapy interventions, including therapeutic exercises, manual therapy techniques, and use of modalities to restore function and alleviate pain.
- Provide patient and caregiver education on therapeutic exercises, mobility techniques, and safety practices to optimize recovery and function.
- Collaboration and Communication
- Collaborate with physicians and interdisciplinary team members to review, adjust, and implement patient care plans as needed.
- Communicate progress, changes in condition, and treatment updates through clear and timely documentation and verbal reporting.
- Participate in care planning meetings with other home health professionals to ensure comprehensive and coordinated care.
- Documentation and Compliance
- Maintain accurate, detailed clinical and progress notes for each patient visit in compliance with agency policies and regulatory standards.
- Submit evaluations, recertifications, and discharge summaries to the physician for approval as required.
- Ensure compliance with all state, federal, and agency regulations governing home health services.
- Supervision and Leadership
- Provide guidance and education to family members and caregivers to support ongoing patient care.
- Participate in quality assurance and performance improvement (QAPI) initiatives as assigned.
- Professional Development
- Stay current on best practices, new techniques, and advancements in physical therapy through continuing education and professional development.
Periodically present findings or techniques to agency staff to foster knowledge-sharing and team growth.
Physical Therapist Requirements:
- Graduation from an accredited Physical Therapy program recognized by the Commission on Accreditation in Physical Therapy Education (CAPTE).
- Successful completion of the National Physical Therapy Examination (NPTE).
- Current licensure in the state(s) of practice (if applicable).
- Minimum of two (2) years of professional experience, preferred.
- Strong clinical and interpersonal skills with the ability to work independently in a home health setting.
- Proficiency in documentation and use of electronic medical records (EMR).
