Indiana Masonic Home Compass Park is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our Home Health Team. Compass Park is a beautiful 320-acre senior living community located in the heart of Johnson County, just minutes from Indianapolis. Our campus offers a vibrant environment throughout all seasons, surrounded by tall trees, scenic walking paths, and a welcoming community atmosphere.
This position is based in Bloomington/Bedford area
General Responsibilities
- Provide professional nursing care as defined by the Nurse Practice Act and under the direction of a Registered Nurse (RN). Communicate appropriate changes in client status or condition to the RN according to agency guidelines.
- Provide skilled nursing visits for clients as directed by the RN Case Manager and follow the established plan of care.
- Communicate information in a timely manner consistent with agency policy and the urgency of the situation through both verbal communication and accurate documentation in the clinical record.
- Promote personal safety and maintain a safe environment for clients and coworkers by recognizing and responding appropriately to potentially unsafe situations. Make recommendations for safety improvements and seek training or assistance when needed.
- Perform duties in compliance with agency policies, professional standards, and state and federal regulations. Maintain strict confidentiality regarding all client and staff information and refrain from discussing identities, conditions, or circumstances with unauthorized individuals.
- Participate in the development, implementation, and evaluation of the agency’s Quality Improvement program and related activities.
- Assist in coordinating home care services, attend required meetings, and follow appropriate reporting guidelines.
- Accurately and promptly document client visits and care provided in the clinical record.
- Recognize and report changes in client condition to the Case Manager and physician as directed.
- Perform additional duties and responsibilities as assigned.

Required Education, Experience, and Skills
- Graduate of an accredited school of nursing
- Current Licensed Practical Nurse (LPN) license in the State of Indiana
- Minimum of one (1) year of experience in an acute care setting or equivalent experience
- Current CPR certification
- Strong written, verbal, and interpersonal communication skills
- Valid driver’s license with a reliable, insured vehicle in accordance with state and agency requirements
- Ability to complete electronic/paperless documentation
