BASIC FUNCTION
The Healthy Start Home Visiting Supervisor provides leadership, oversight, and strategic direction for the Coordinated Intake & Referral (CI&R), Parents as Teachers (PAT), and Health Education programs. This position supervises staff, ensures program fidelity, monitors performance outcomes, and supports continuous quality improvement.
The Healthy Start Home Visiting Supervisor serves as the primary liaison to Cleveland Clinic Indian River Hospital to ensure coordination of services, compliance with clinical and referral protocols, and seamless communication between hospital staff and Healthy Start programs. This role strengthens hospital-community partnerships to improve maternal and infant health outcomes.
The Healthy Start Home Visiting Supervisor collaborates with internal leadership, community partners, healthcare providers, and coalition staff to ensure high-quality, evidence-based services for pregnant women, families, and children in Indian River County.
SUPERVISORY ACCOUNTABILITY
This position directly supervises program staff within:
- Coordinated Intake & Referral (CI&R)
- Parents as Teachers (PAT)
- Health Education
Supervisory responsibilities include hiring support, onboarding, training, performance evaluation, corrective action, workflow oversight, and staff development. The Supervisor ensures staff maintain certification and fidelity to required evidence-based models and funding guidelines.
NATURE AND SCOPE
The Supervisor works closely with:
- Pregnant women and families
- Hospital administration and maternity unit staff
- Community partners and referral agencies
- Healthy Start leadership and coalition staff
- Funding entities and quality assurance personnel
The role requires balancing direct program oversight, data monitoring, staff supervision, hospital coordination, and strategic planning.
PRINCIPAL ACCOUNTABILITIES
Program Leadership & Oversight:
- Provide leadership and direction for CI&R, PAT, and Health Education programs.
- Ensure adherence to evidence-based model requirements and grant deliverables.
- Monitor client caseloads, service delivery timelines, referral completion, and engagement benchmarks.
- Participate in strategic planning and management meetings.
Hospital Liaison Responsibilities:
- Serve as primary point of contact between Healthy Start programs and Cleveland Clinic Indian River Hospital.
- Maintain strong working relationships with OB, labor & delivery, and PIWH
- Monitor and streamline hospital referral processes.
Staff Supervision & Development:
- Provide bi-monthly individual supervision to staff.
- Conduct annual evaluations and ongoing performance monitoring.
- Support professional development and required certifications.
Quality Assurance & Data Management:
- Monitor required program indicators and funding objectives.
- Submit monthly and quarterly QA/QI reports.
- Ensure accurate documentation in required data systems.
Community Collaboration & Outreach:
- Strengthen partnerships with healthcare providers and community agencies.
- Represent programs at community events.
Training & Education:
- Ensure staff are trained in policies and evidence-based practices.
- Support implementation of required screenings.
- Coordinate internal and community-based trainings.
Operational Support:
- Support staff coverage during absences.
- Maintain a safe and supportive work environment.
- Perform other duties as assigned.

- Experience collaborating with hospital systems strongly preferred.
- Experience with quality assurance and performance monitoring.
- Proficiency with MS Office and database systems required.
Pay: $50,000.00 - $60,000.00 per year
