JOB SUMMARY
The Clinician provides clinical support, working in multiple systems, including family/home care providers homes, community day/vocational programs, school. The Clinician identifies treatment guidelines and develops behavior plans, provides training for same, tracks progress, presents to human rights committees as necessary.
LOCATION – primary areas of Claremont/Concord/Manchester/Nashua, all in NH
DUTIES/RESPONSIBILITIES/ATTRIBUTES
- Demonstrates a person-centered approach, treating all clients, families and collaterals in a non-judgmental and respectful manner while building on their strengths
- Provides group and/or individual training and support to staff, the individuals we support, home care providers
- Develops and maintain timely assessments, progress notes and other necessary documentation
- Facilitates treatment meetings as applicable
- Evaluates effectiveness of treatment plans on an ongoing basis
- Requires working a flexible schedule including shared rotating on-call responsibilities for nights and weekends and 24-hour crisis response.
- Develops and maintains positive relationships with all parties
- Maintains organized up to date case files in accordance with MEDICAID requirements
- Participates in companywide key initiatives
EDUCATION & EXPERIENCE REQUIREMENTS
- Master’s Degree in related field
- Demonstrated successful development/implementation/training of treatment plans
- Relevant experience with adults with intellectual disabilities in community-based settings.
SKILLS AND ABILITIES
- Experience with medically complex individuals a plus.
- Excellent verbal and written communication skills.
- Strong interpersonal and organizational skills.
