Job Summary:
We are currently seeking a In-Home Community Support Specialist to join our team in providing essential care and support to individuals with disabilities. As a In-Home Community Support Specialist, you will play a crucial role in assisting clients (working one- on-one) with various daily activities, promoting their well-being, and ensuring a safe and nurturing environment.
Hours: M-F between 8am-6pm primarily.
Responsibilities:
- Implement individual service needs which promotes independence and the achievement of individual outcomes as stated in the Individual Support Plan (ISP)
- Develops, plans, and implements goal/skill drive programs/activities for the individual.
- Prepares and completes all forms of documentation as required.
- Provides personal care assistance as necessary in a safe and consistent manner consistent with guidelines stated in the individual support plan in order to facilitate the individual's ability to participate in program activities.
Requirements:
- High School diploma or GED
- Minimum of one year experience working with people with disabilities, plus experience supporting adults in their home, adult day program, or adult recreational setting preferred.
- Must be able to lift 50 lbs independently and pass a company physical and criminal background check.
- Must have a valid driver's license to drive company vehicle
- Excellent communication skills and empathy towards clients
