Responsibilities
- Conduct comprehensive patient assessments, including vital signs, physiology evaluation, and medical history review, to determine individual care needs.
- Develop and implement tailored case management plans for diverse patient populations.
- Coordinate discharge planning by facilitating communication between hospital staff, outpatient clinics, and community resources to ensure smooth transitions of care.
- Manage utilization review and management processes to optimize resource use while maintaining high-quality patient care standards.
- Monitor diagnostic evaluations and medical records to track progress and adjust care plans accordingly.
- Collaborate with healthcare providers across emergency medicine, hospital medicine, trauma centers (Level I & II), infectious disease units, and outpatient clinics to ensure comprehensive patient support.
- Educate patients and families on medical terminology, medication management, and post-discharge instructions to empower self-care and adherence.
- Maintain strict compliance with HIPAA regulations to protect patient privacy and confidentiality at all times.
Experience
- Proven experience in critical care environments such as ICU or emergency room settings with a strong understanding of physiology and acute care protocols.
- Prior work in Level I or Level II trauma centers or trauma-related case management is highly desirable.
- Experience managing complex cases involving infectious diseases, behavioral health issues, geriatrics, pediatrics (PICU), or medical-surgical units.
- Familiarity with hospital medicine practices, including inpatient and outpatient clinic operations.
- Knowledge of managed care principles, utilization management strategies, discharge planning procedures, and post-acute care coordination.
- Strong background in nursing with expertise in patient assessment, diagnostic evaluation, vital signs monitoring, and medical terminology.
- Ability to navigate medical records systems efficiently while ensuring compliance with HIPAA standards.
- Excellent communication skills for collaborating with multidisciplinary teams across hospital experience settings—acute care to outpatient clinics—and engaging patients effectively.
