What This Role Offers Flexible schedule (15-30 hours per week)
Primarily weekday hours
No shift work, no overnight requirements

What You'll Do
Conduct scheduled client visits (weekly / bi-weekly)
Complete new client assessments and reassessments
Oversee and update individualized care plans
Provide guidance and reassurance to family caregivers
Support and mentor CNA caregivers
Communicate with physicians as needed
