Position Summary
The Social Services Lead works to nurture a trauma competent and recovery-oriented milieu on both the Inpatient Unit. Promotes the development of and supports ongoing evidenced based treatment. The Social Services Lead will complete psychosocial assessments, provide discharge planning, ensure continuity of care and guide therapeutic groups, including seeking to engage family and social supports of individuals served. The Social Services Lead will help nurture external relationships and will be credentialed with supporting hospitals as required. The Social Services Lead will also provide clinical supervision to non-nursing Inpatient staff, including social workers, interns, social services assistants, activity therapists, recovery coaches and recovery technicians. The Social Services Lead will support the mission of both IPU and Unity House, understanding the workings of both floors and being of assistance as needed at Unity House.

Essential Duties And Responsibilities

- Incorporates the six principles of trauma informed care throughout all aspects of their work.
- Provides direct treatment according to agency policy and procedures
- Works to support a climate that is committed to there being many paths to recovery and trauma-informed competency.
- Facilitates transfer to State operated facilities, nursing homes, or other placements by making all necessary arrangements (date of transfer, transportation, paperwork, transfer packets, etc.) in tandem with IPU Treatment Team.
- Acts as a liaison with the courts as needed, facilitates necessary documentation completion for involuntary commitments.
- Completes patient registration and schedules follow up appointments for patients with a master’s level therapist and psychiatric provider in accordance with CMS regulations.
- Conducts discharge conferences with client, family, or collateral professionals for continuity of care and transition of services as requested.
- Cultivates and utilizes a broad and effective referral network to assist with the external placing of clients when indicated.
- Develops linkage systems with community resources along with the treatment team to ensure that discharged clients have a well-planned and designed community support system.
- Serves as liaison with area offices to assure continuity of care.
- Responds to inpatient clinical emergencies as needed.
- Serves as back up for utilization review; contacts insurance companies for verification and authorization of benefits and pre-certification, as necessary.
- Ensures all policy, procedure and regulations of the hospital are met
- Monitors the execution of evidence-based practices by the social service team
- Engages with agency and outside referral sources
- Fosters an engaging environment, provides regular clinical interventions and associated documentation.
- Fosters team cohesion among social services and the general IPU team.
- Providers clinical supervision and associated documentation
- Ensures staff are trained according to agency policy and procedures
- Completes timely reports/documentation
- Has an active Indiana License, or license eligible as Licensed Clinical Social Worker (LCSW or LSW).
- Credentials with local hospitals as required
- Must be willing to work all shifts as position requires.

- Willingness to partner with residents regardless of stage of change, seeking to demonstrate compassion and working to guide and evoke change.
- Knowledge of social determinants of health and recovery capital.
- Skills in advocacy, organization, and communication.

- Minimal travel may be required for trainings or meetings
