Job Overview
An LPN in an Intellectual/Developmental Disability (IDD) group home provides skilled nursing care, medication administration, and assistance with daily living activities to individuals with IDD. This role requires collaboration with other healthcare professionals, documentation, and training of other staff members. We are seeking a dedicated and compassionate LPN's to oversee the daily nursing operations of our residential group home.

Inclusive Care Community provides group home residential and nursing services for clients with intellectual & developmental disabilities (IDD). Direct care will be required to meet the needs of the program. This position also entails providing LPN nursing services in the operation of the residential group home program. It includes supporting those activities related to organizing and implementation of policies and procedures of Inclusive Care Community and mandates of program regulated by licensure and the Department of Medical Assistance Services. The person in this position reports directly to the Executive Director or their delegate.

Responsibilities

  • Providing Skilled Nursing Care:Administer medications, monitor vital signs, perform wound care, and assist with other medical procedures.
  • Assisting with Daily Living Activities:Helping with hygiene, dressing, eating, and other activities of daily living.
  • Monitoring and Assessing Clients:Observing for changes in health status, gathering data, and reporting concerns to supervisors or other healthcare professionals.
  • Developing and Implementing Care Plans:Participating in the development and implementation of individual care plans, ensuring they are aligned with the needs of the residents.
  • Documentation:Maintaining accurate records of care provided, including medication administration records and progress reports.
  • Training and Supervision:Providing training and support to other staff members, including Certified Nursing Assistants (CNAs), on basic nursing skills and procedures.
  • Communication and Collaboration:Effectively communicating with residents, families, and other healthcare professionals to ensure a coordinated approach to care.
  • Safety and Emergency Response:Responding to emergencies and implementing appropriate protocols to ensure the safety of residents and staff.
  • Other duties as assigned.

  • Experience in dementia care and working with individuals with developmental disabilities.
  • Ability to monitor vital signs effectively and respond to medical emergencies.
  • Excellent organizational skills with attention to detail in meal preparation and caregiving tasks.
  • Strong interpersonal skills to build relationships with residents, families, and staff members.
  • Compassionate demeanor with a genuine desire to improve the quality of life for seniors.

Pay: $30.00 - $32.00 per hour
