Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor and observe patients’ health status, noting changes in condition and vital signs such as blood pressure, pulse, and temperature
- Support medication administration following prescribed guidelines while maintaining accurate medical documentation
- Utilize Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to record patient information securely and efficiently
- Provide specialized care for individuals with dementia, Alzheimer’s disease, or other memory impairments, including memory care techniques
- Assist with catheter care, wound care, and hospice support as needed to ensure comfort and safety
- Operate medical equipment such as Hoyer lifts safely and effectively to assist with patient transfers
- Support patients with mobility challenges using assistive devices and proper lifting techniques while adhering to HIPAA regulations for privacy and confidentiality
- Prepare nutritious meals and assist with feeding while considering dietary restrictions or medical needs
- Perform light housekeeping tasks including cleaning, laundry, and maintaining a safe environment in accordance with health standards
Requirements
- Proven experience working as a Home Health Aide or in assisted living, nursing home, or long-term care settings
- Knowledge of medical terminology, medication administration protocols, and vital signs assessment
- Certification in CPR (Cardiopulmonary Resuscitation) is required; additional certifications such as hospice care or dementia training are a plus
- Experience working with individuals with disabilities or developmental disabilities preferred
- Ability to operate EMR/EHR systems accurately for documentation purposes
- Strong patient observation skills to identify changes in condition promptly
- Compassionate attitude with excellent communication skills to build trusting relationships with clients and their families
- Valid driver’s license and reliable transportation are often necessary for home visits
