As a Home Health Aide, you will work closely with clients, families, and healthcare professionals to deliver high-quality care that promotes independence and comfort.
Responsibilities
- Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor and record vital signs such as blood pressure, pulse, temperature, and respiration rates using Electronic Health Record (EHR) systems
- Support medication administration following prescribed protocols while maintaining strict compliance with HIPAA regulations for patient privacy
- Provide specialized care for individuals with dementia, Alzheimer's disease, or other memory-related conditions, including memory care techniques and safety measures
- Perform patient observation to identify changes in health status and communicate findings promptly to supervising nurses or healthcare providers
- Assist with medical procedures such as catheter care, wound care, and Hoyer lift transfers safely and effectively
- Support patients with hospice or palliative care needs by offering emotional comfort and managing symptoms compassionately
- Perform light housekeeping tasks including cleaning, laundry, meal preparation, and ensuring a safe living environment for clients
- Document all patient interactions accurately in medical records and ensure compliance with medical documentation standards
- Work with EMR (Electronic Medical Records) systems to update patient information efficiently and securely
Qualifications
- Proven experience as a Home Health Aide or in a similar caregiving role within assisted living facilities, nursing homes, or home health settings
- Knowledge of medical terminology and procedures related to patient care and safety standards
- Certification in CPR (Cardiopulmonary Resuscitation) is required; additional certifications such as medication administration or hospice care are a plus
- Experience working with individuals with developmental disabilities or disabilities requiring long-term care is highly valued
- Ability to perform patient observation accurately and respond swiftly to changing health conditions
- Familiarity with EHR/EHR systems for medical documentation and record keeping
- Strong communication skills to collaborate effectively with healthcare teams and families
- Ability to lift patients safely using equipment like Hoyer lifts when necessary
- Valid driver’s license and reliable transportation preferred for home visits
