Physical Therapist I - Outpatient Pediatric Therapy

Employment Type: Full time Shift: Day Shift Description: Hours: Monday - Friday; no weekends required JOB SUMMARY Assesses and treats pediatric patients referred to the Physical Therapy department. Documents patient evaluation, plan of care, progress and discharge status. JOB DUTIES 1. Knows and adheres to all laws and regulations pertaining to patient health, safety, medical information. Complies with regulatory agency (Medicare, JCAHO, CARF etc.) standards and regulations. 3. Conducts evaluations consistent with and appropriate for patient diagnosis. Designs a treatment plan that is goal oriented based upon interpretation of evaluation results and patient’s personal goals. Establishes goals that are functional, measurable and attainable. Performs re-evaluations per department criteria with modifications/updating of goals as dictated by patient’s progress 4. Assists patient towards goal achievement through effective treatment plan and interventions. Administers individual treatment procedures. Develops home exercises and self treatment plans for the patient 5. Provides education to patient and family, including diagnosis and prognosis of therapy. Recognizes adverse responses or contradictions to therapy and adjust treatment accordingly. 6. Makes timely and appropriate discharge plans and recommendations for the anticipated needs of the patient. 7. Responsible for the accurate and thorough completion of, evaluation reports, progress notes/flow sheets discharge summary and any additional required documentation (e.g. consent, staffing reports, rounds, attendance agreements, patient and family education needs assessment form) in a specified time frame. Documents the occurrence of education by recording occurrence(s) of education and /or home exercise programs provided and reports patient/family return demonstration and /or comprehension of training provided. Adheres to documentation requirements by regulatory agencies. (e.g. records duration of specific treatment procedures in minutes). Submits daily charges that accurately reflect documentation within a specified time frame and assures that documentation is complete prior to discharging patient. 8. Participates in staff orientation, patient staffing and team meetings. Makes appropriate recommendations and referrals for additional services. Consults with other professionals and/or appropriately seeks assistance from co-workers as needed. Maintains flexible hours of availability to meet department needs. Engages patient using interpersonal dynamics to facilitate and effect therapeutic changes. Participates in case management needs as per requirements in individual departments. 9. Maintains established departmental productivity standards. 10. Actively participates in departmental initiatives such as Performance Improvement Initiatives, CARF, JCAHO and State Board of Health readiness. 11. Provides direction to Physical Therapy Assistants by delegating appropriate patients, reviewing plan of care and providing clinical support as necessary. Provides day-to-day direction to rehab techs and volunteers. 12. Participates in student program as appropriate and based on level of experience and expertise. 13. Attends all mandatory in-services and meetings and other in-services that are related to area of responsibility. Provide in-service for staff after attending courses outside the organization. 14. Adheres to and maintains all necessary competencies required for job responsibilities. 15. Performs other duties consistent with purpose of job as directed. JOB SPECIFICATIONS AND CORE COMPETENCIES Education: Requires professional education culminating in a minimum of a bachelor’s degree in Physical Therapy from an accredited academic program. Licensure: Must be licensed in the State of Indiana as a Physical Therapist. Experience: Prefer previous experience in a physical therapy clinical setting but will accept a new graduate from an accredited college. Other Job Requirements: Excellent communication skills, both verbal and written are required. Must be able to adjust to varying treatment loads and schedules. Requires flexible work schedule including weekends. Considerable initiative, judgment and problem solving required.
