Employment Type: Full time Shift: Description: Develops, implements, and maintains systems to address the pharmacy quality metrics of the Population Health Department and is driven to meet and exceed customer expectations. Holds self and team accountable for achieving the highest level of patient care, quality metrics, customer service, and employee satisfaction. Recognizes the contribution for desired results. Assists the director in selecting, developing, and maintaining a team of proficient staff JOB SPECIFICATIONS AND CORE COMPETENCIES Education: Requires the knowledge of theories, principles, and concepts acquired through the completion of the degree of a Doctorate of Pharmacy. Licensure: Must hold a valid Indiana pharmacy license or be a graduate of an accredited school or college of pharmacy and licensed in the state of Indiana as a pharmacy intern. Experience: Candidate has PGY-1 Residency Training with rotations in Population Health or Ambulatory Care. In lieu of residency training, candidates have pharmacist experience in population health. PGY2 residency preferred. BCPS or BCACP Certification preferred. Other Job Requirements: Associates must demonstrate independent critical thinking skills and the ability to appropriately and effectively convey concepts to a diverse population of people with varied educational experience.
