Duties
- Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility support using equipment like Hoyer lifts when necessary.
- Monitor and record vital signs including blood pressure, pulse, respiration, and temperature to track health status accurately.
- Support medication administration following prescribed instructions while maintaining strict compliance with HIPAA regulations to protect patient privacy.
- Observe and document patient conditions, behaviors, and responses to care interventions using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems for accurate medical documentation.
- Provide specialized care for patients with dementia or Alzheimer's disease, including memory care strategies and behavioral management techniques.
- Assist with medical procedures such as catheter care and hospice support to ensure comfort and safety.
- Prepare nutritious meals and assist with feeding as needed, ensuring dietary restrictions are followed precisely.
- Perform light housekeeping tasks like cleaning, laundry, and maintaining a safe environment to promote overall well-being.
- Use medical terminology confidently when communicating with healthcare teams or documenting patient information.
- Support patients in community settings or assisted living facilities, fostering independence while offering necessary supervision and assistance.
Qualifications
- Proven experience in home health care or long-term care environments; nursing home experience is highly valued.
- Strong knowledge of patient observation techniques and ability to recognize changes in condition promptly.
- Familiarity with EMR/EHR systems for documentation purposes; prior experience working with medical records is preferred.
- Demonstrated ability to assist with ADLs efficiently and compassionately in various settings including assisted living or hospice care.
- Working knowledge of medical terminology relevant to home health services.
- Certification in CPR is required; additional certifications such as medication administration, catheter care, dementia or Alzheimer’s care are advantageous.
- Experience working with individuals with developmental disabilities or disabilities requiring specialized support is a plus.
- Ability to operate equipment like Hoyer lifts safely and effectively while maintaining patient dignity.
- Excellent communication skills for collaborating with healthcare teams and family members while respecting patient confidentiality under HIPAA guidelines.
