Position Summary:
The Care Coordinator / Client Services Coordinator is responsible for coordinating high-quality client care services while ensuring IHPS standards are consistently met. This role manages client intake, caregiver scheduling, Medicaid authorization processes, compliance, quality assurance, and ongoing communication with clients, families, caregivers, and referral sources. The Care Coordinator ensures all services are delivered in accordance with individualized care plans, agency policies, and applicable federal, state, and local laws and regulations.
Essential Duties and Responsibilities:
- Establish and maintain professional relationships internally and externally
- Coordinate and manage client care services in accordance with individualized care plans
- Conduct client assessments, start-of-service procedures, and ensure timely service initiation
- Manage caregiver scheduling, staffing assignments, and coverage for call-outs, schedule changes, and emergencies
- Oversee caregiver endorsements to new cases by communicating client needs, family expectations, and ensuring caregiver competency
- Serve as the primary liaison between clients, families, caregivers, case managers, and referral partners
- Communicate regularly with clients and families regarding care plans, service updates, and client concerns
- Monitor caregiver performance through supervision, coaching, intervention, and recognition
- Conduct routine quality assurance visits to assess caregiver performance and client satisfaction
- Ensure all client and caregiver records, notes, and documentation are accurate, complete, and submitted on time
- Ensure confidentiality and compliance with agency policies, Medicaid requirements, and state regulations
- Identify and report changes in client condition or service concerns to supervisors or appropriate healthcare professionals
- Respond promptly and professionally to client, family, and caregiver concerns, ensuring timely resolution
- Assist with recruitment support, onboarding, orientation, training, and competency certification for new caregivers
- Manage payroll-related scheduling accuracy and staffing efficiency
- Complete required training on Medicaid authorization approvals, renewals, eligibility verification, and service compliance for Medicaid clients
- Coordinate with Medicaid programs, MCOs, case managers, and referral agencies to obtain and maintain service authorizations
- Monitor authorization expiration dates and ensure continuity of care for Medicaid clients
- Oversee compliance with risk management protocols, incident reporting, corrective actions, and follow-up procedures
- Provide hands-on client care or shift coverage when necessary
- Participate in all required meetings, in-services, audits, and quality reviews
- Support company growth through networking events, marketing initiatives, and community relationship building
- Remain available during all operational hours as needed
- Perform additional duties assigned by the Administrator
