Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor patients’ health status by observing vital signs, managing medication administration, and reporting changes promptly
- Prepare nutritious meals tailored to individual dietary needs and assist with feeding as necessary
- Maintain accurate care plans and document patient progress in compliance with HIPAA regulations
- Utilize specialized equipment such as Hoyer lifts or wheelchairs to ensure safe transfers and mobility support
- Provide companionship and emotional support to promote mental well-being and reduce feelings of isolation
- Perform light housekeeping tasks including cleaning, laundry, and maintaining a safe living environment
Qualifications
- Prior experience in home health, assisted living, nursing home, or group home settings preferred
- Knowledge of patient observation techniques and ability to monitor vital signs effectively
- Certification in CPR and First Aid required; additional certifications such as Alzheimer's or dementia care are a plus
- Experience working with individuals with developmental disabilities, or memory care needs highly desirable
- Familiarity with HIPAA regulations to ensure confidentiality and privacy of patient information
- Ability to lift heavy objects safely and assist with patient transfers using equipment like Hoyer lifts
