- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor patient health by checking vital signs such as blood pressure, pulse, and temperature regularly
- Provide medication reminders and support medication administration following care plans and HIPAA regulations
- Prepare nutritious meals tailored to individual dietary needs and assist with meal planning and cooking
- Support clients with mobility using equipment such as Hoyer lifts or other assistive devices; perform heavy lifting when necessary
- Maintain detailed documentation of patient observations, care provided, and any changes in condition to ensure accurate care plans
- Conduct patient observation to identify potential health concerns or safety issues promptly
- Perform light housekeeping tasks such as cleaning, laundry, and maintaining a safe living environment
- Support clients with social activities and companionship to promote mental well-being
- Follow all safety protocols including CPR and First Aid procedures to ensure client safety at all times
Qualifications
- Prior experience in home & community care or assisted living environments is highly preferred
- Knowledge of dementia care, Alzheimer's care, memory care, hospice care, and working with individuals with developmental disabilities or autism is advantageous
- Certification in CPR and First Aid required; additional certifications such as HIPAA training are a plus
- Experience with patient monitoring tools and techniques for vital signs assessment
- Ability to perform heavy lifting safely; familiarity with Hoyer lifts or similar equipment is beneficial
- Strong communication skills to effectively collaborate with clients, families, and healthcare professionals
- Compassionate attitude with a commitment to providing respectful, person-centered care
- Valid driver’s license and reliable transportation preferred for community-based visits
