Job Summary
In-Home Care Connection & Hospice is seeking a a compassionate Medical Social Worker to join our team. In this vital role, you will provide comprehensive psychosocial support to patients and their families, helping them navigate complex medical conditions, emotional challenges, and social needs. Responsibilities
- Conduct thorough patient assessments to identify psychosocial needs, including mental health status, family dynamics, and social determinants affecting health outcomes.
- Develop and implement individualized care plans that incorporate behavioral health strategies, counseling techniques, and resource coordination.
- Collaborate with multidisciplinary teams—including physicians, nurses, therapists, and social service providers—to coordinate comprehensive patient care plans.
- Engage in case management activities such as discharge planning, home & community care coordination, assisted living arrangements, and hospice or geriatrics services.
Requirements
- Master’s degree in Social Work (BSW or MSW) from an accredited institution; Social Work Assistant with BS degree in Psychology or Social Work with home health/hospice/long term care experience will be considered.
- Valid drivers license and reliable transportation
Job Type: Part-time
Base Pay: $28.00 - $32.00 per hour
Expected hours: 10 – 30 per week
