Job Title: Designated ManagerSummary
The Designated Manager oversees all licensed 245D services, ensures the agency is compliant with Minnesota statutes, and makes sure staff deliver services exactly as written in each person’s CSSP, Addendum, and support plan. This role supervises Designated Coordinators, monitors documentation, handles incident reviews, and keeps the program audit-ready.
Key Responsibilities
Program Oversight
- Ensure all 245D requirements are followed across IHS w/Training, IHS Without Training, Homemaker, Night Supervision, and ICS.
- Oversee development, implementation, and monitoring of each person’s CSSP Addendum, IAPP, and support strategies.
- Review DC work to ensure documentation and follow-through are accurate and timely.
Compliance & Quality
- Maintain full DHS compliance: policies, orientation, annual training, internal reviews, VA reporting, rights restrictions, emergency planning.
- Conduct internal audits — service notes, MARs, training records, incident reports, billing accuracy, etc.
- Lead 245D annual internal review process and submit findings/corrections.
Staff Leadership
- Supervise Designated Coordinators and provide direction on complex client issues, critical incidents, and service changes.
- Ensure staff training is completed and documented: 245D orientation, vulnerable adult, mandated reporter, medication assistance, rights, etc.
Client/Team Coordination
- Communicate with case managers, guardians, teams, and counties regarding service issues, risk concerns, and plan changes.
- Approve and oversee admissions, discharges, transitions, and terminations.
Incident & Risk Management
- Oversee maltreatment reporting procedures and ensure mandated timelines are met.
- Review incidents, develop corrective actions, and ensure staff follow through.
Qualifications
Must meet MN 245D requirements for a Designated Manager:
- Bachelor’s degree in human services field OR
- Associate degree 2 years experience OR
- Diploma 3 years experience OR
- 50 hours of education 4 years experience
- Strong knowledge of 245D licensing, positive supports, person-centered planning, and HCBS waiver requirements.
- Experience supervising staff and managing programs.
- Strong organization, follow-through, and documentation skills.
Preferred Skills
- Experience with IDD/MH/DD/brain-injury populations.
- Familiarity with MAR/MOR systems, EHRs, and case management communication.
- Ability to problem-solve quickly and keep the agency compliant under pressure.
Pay: $25.00 - $30.00 per hour
