Description:
The Coordination Manager oversees in-home care services, ensuring high-quality patient care and compliance with healthcare standards. This role is responsible for managing a team of home care coordinators, maintaining efficient scheduling operations, and driving process improvements. The Coordination Manager serves as a key escalation point for patient, caregiver, and provider concerns while promoting continuous improvement in service delivery and operational performance.
What You Will Do:
- Supervise daily operations of the Coordination Department to ensure accurate and timely caregiver scheduling aligned with client needs and regulatory requirements
- Lead, coach, and develop a team of coordinators through performance management, training, and ongoing feedback
- Oversee case assignments, schedule changes, and urgent coverage needs to ensure continuity of care and patient safety
- Ensure caregiver compliance with DOH and agency standards prior to placement, including certifications and supervisory requirements
- Act as the primary escalation point for scheduling conflicts, caregiver concerns, and client service issues, ensuring timely resolution and proper documentation
- Collaborate cross-functionally with Intake, Compliance, HR, and external partners such as MLTC plans and hospitals
- Audit scheduling and caregiver records within systems like HHAeXchange to ensure accuracy and compliance
- Monitor team productivity and KPIs, identify gaps, and implement workflow improvements and quality initiatives
- Prepare and present reports on staffing coverage, service metrics, and departmental performance
- Foster a collaborative, accountable, and high-performing team environment

What You Will Bring:
- 4 years of experience in home care coordination or healthcare scheduling, including at least 1 year in a leadership role
- Strong knowledge of New York State DOH regulations, caregiver compliance requirements, and Medicaid/MLTC processes
- Experience with scheduling platforms such as HHAeXchange or similar systems
- Demonstrated leadership, team management, and staff development skills
- Strong organizational, time management, and problem-solving abilities
- Ability to manage scheduling challenges such as caregiver callouts and coverage gaps
- Excellent written and verbal communication skills with diverse stakeholders
- Bilingual capability (English plus Spanish, Russian, or Cantonese) is a plus
