Position Summary
As a Part-Time Nurse Practitioner (NP) with Impact Health PC in Shelton, CT, you will provide Medical/Primary Care to residents within skilled nursing facilities.
You will assess and manage acute and chronic conditions, develop treatment plans, and collaborate with physicians, facility staff, and interdisciplinary care teams.
Key Responsibilities

Assess, diagnose, and manage acute and chronic medical conditions.
