Position Summary
As a Nurse Practitioner (NP) with Impact Health PC in Augusta, ME, you will provide comprehensive care to patients within skilled nursing facilities. You will assess and manage acute and chronic conditions, develop treatment plans, and collaborate with physicians, facility staff, specialists, and interdisciplinary care teams.
Key Responsibilities
- Assess, diagnose, and manage acute and chronic conditions.
- Develop evidence-based treatment plans.
- Complete timely clinical documentation and place appropriate orders.
