Impact Behavioral Health Partners seeks a qualified and experienced Clinical Case Manager to serve as a member of a multidisciplinary team that provides services to residents in Housing Authority of Cook County (HACC) buildings across north suburban Cook County. This Clinical Case Manager will provide a range of strength-based, person-centered services that empower participants to maintain their independence and housing stability. This role will require increased outreach efforts and close collaboration with HACC property managers and Impact’s HACC team to ensure service delivery. This job is suited for a proactive self-starter who can learn new things quickly, be responsible with time management, and work independently with limited supervision. Excellent communication skills and organizational skills are needed. Flexibility to adapt to unpredictable situations is essential. Competency with computers is necessary, but this job is meant for a people person who would rather not sit behind a desk all day. This position reports to the Clinical Supervisor who oversees the HACC program. Location:The person in this role will be required to spend at least 75% of their time working on site at HACC buildings in north suburban Cook County. This role will be assigned to work at HACC buildings in Skokie and Niles. Some meetings and administrative work will be completed via video conferencing.The ability to work remotely from home may be possible at times but cannot be guaranteed. Schedule:Full-time, 37.5 hours per week.Flexible scheduling may be available to accommodate employee needs outside of work but cannot be guaranteed.The person in this role may need to occasionally be available to change their schedule to accommodate participants with special needs or to assure adequate clinical team coverage.Salary:Starts at $45,000 per year.Performance-based incentives may be possible. Independently manage a caseload of 25 to 35 individuals that are regularly engaged. Manage check ins and outreach for other individuals that are disengaged.Manage difficult or emotional situations with participants, respond to participant needs, solicit feedback from participants to improve services, and respond to participant requests for service and assistance. Provide a range of services that include but are not limited to case management, resource connection, team-based case coordination, crisis intervention, skills training in activities of daily living, and discharge planning.Assist participants in applying for and maintaining public benefits such as Medicaid, SNAP, Social Security, etc.Conduct outreach and engagement activities such as presentations, leading groups, and attending community meetings. Fully promote and participate in the integration of Housing First, Harm Reduction, and Trauma Informed Care approaches across all services. Complete documentation of all relevant participant activities, such as progress notes, initial and ongoing assessments (IM CANS, LOCUS), crisis assessment/safety plans, critical incident reports, etc. in a timely manner. Meet all established caseload and billing requirements. Additional Job Functions:Adhere to Impact’s policies and procedures.Adhere to the expectations and regulations of various funding sources. Demonstrated social services experience with persons with a chronic mental illness, preferably in a housing or multidisciplinary setting.1-2 years relevant experience preferred, internships or employment.Demonstrated experience in and commitment to providing services from a harm reduction approach. Demonstrated ability to provide clear and effective communication both verbal and written. Demonstrated ability to identify and resolve problems skillfully and in a timely manner, both individually and working with others to solve problems as a group.Working knowledge of pertinent software, such as Microsoft Office, Google Workspace and Electronic Health Records software. Must have a valid driver’s license.Must have a private office area and reliable internet access while working remotely.Must have access to a reliable carMust obtain and maintain a $100,000/$300,000 bodily injury and liability auto insurance policy. Proof of insurance must be provided at least annually. Physical Requirements:Ability to twist, turn, bend, climb stairs, and lift as required to perform the duties associated with this job.
