IMPACCT Brooklyn is seeking a skilled and compassionate Case Manager Supervisor to lead our Case Management team in delivering exceptional services to clients. In this role, you will ensure that all case management activities meet professional standards, fulfill the requirements of our funding partners, and reflect the organization’s mission and values. You will manage program staff at two supportive housing buildings, provide guidance and supervision to the team, and share responsibility for day-to-day program operations. The ideal candidate will possess strong clinical and organizational abilities, excellent communication skills, and proficiency with basic computer applications. Experience working with multiple agencies and a strengths-based, hopeful approach to supporting individuals with persistent mental illness are essential. This position reports directly to the Director of Supportive Housing and offers an opportunity to make a meaningful impact on our clients' lives and the quality of our programs.
Essential Duties and Responsibilities
- Ensure delivery and documentation of Case Management services in compliance with professional standards, funding requirements, and agency policies.
- Supervise direct reports, oversee internal and external reporting, and ensure the quality of supportive services, community outreach, screening, assessment, and eligibility reviews.
- Provide on-site supervision at Gibb Mansion Residence and Lexington Senior Residence.
- Lead and monitor clients through the intake, orientation, and assessment process to identify service needs.
- Develop mutually agreed-upon service plans with clients and monitor progress through case conferences, staff meetings, and regular reviews.
- Responsible for the completion of annual psychosocial assessments and reassessments for all program clients.
- Maintain accurate, confidential, and timely documentation of all case management activities.
- Develop and maintain positive relationships with clients, agency partners, community resources, and state/city liaisons.
- Provide crisis intervention and substance abuse consultation, training, and case conferencing to the social service team as needed.
- Actively participate in interdisciplinary case conferencing and maintain strong collaboration with social service staff to recommend best practices.
- Work with direct reports to create an annual calendar of recreational, social, educational, and skill-building activities that promote client inclusion and integration.
- Collaborate with the Director to coordinate and supervise the daily operations of the Social Service Department.
- Assist in developing and implementing departmental plans, goals, and budgets; monitor expenses as required.
- Participate in job-related training and other duties as assigned.
Minimum Qualifications
- Must possess one of the following: an LMSW; a Master’s Degree in Social Work with the expectation of obtaining an LMSW within six months of hire; or a Master’s Degree in Mental Health Counseling along with a valid LMHC or equivalent New York State license.
- Minimum 3 years of experience working with diverse populations, including older adults and individuals with behavioral health and mental health challenges
- At least 2 years of supervisory experience; residential setting experience preferred
- Over 2 years of experience in developing and facilitating substance use and harm reduction programs, including training, psychoeducation, and both individual and group work
- Familiarity with the Housing First model and person-centered, trauma-informed, and solutions-focused approaches
- Strong leadership and interpersonal skills
- Ability to work independently and thrive in a fast-paced environment
- Excellent written and verbal communication skills in English required
- Proficient in MS Office and related computer applications
- Bilingual in Spanish, Mandarin, or Cantonese is a plus
Supervisory Responsibilities
This position provides direct supervision to the Case Management staff and is responsible for performance management, training, and onboarding of team members.
Salary Rages: $65,000.00 to $75,000.00

FLSA & EMPLOYMENT STATUS
Exempt, full-time position. The typical work schedule is Monday through Friday, from 9:00 a.m. to 5:00 p.m. Occasionally, evening or weekend work may be required, including overtime, participation in special program events, and on-call hours to respond to urgent situations.
PHYSICAL DEMANDS AND WORK ENVIRONMENT
This position operates in both a professional office environment and in the field. Can regularly travel to tenants’ residences and to community-based appointments and partner organizations throughout the service area, and conducts home visits and unit inspections that may involve walking, climbing stairs, and entering residential buildings with varied conditions. The physical demands described here represent those that an employee must meet to perform the essential functions of this position successfully. The role requires the ability to stand or sit for extended periods, work on a computer, move throughout the office and the community, and occasionally lift materials weighing up to 25 pounds. Interaction with clients and staff fosters a collaborative and supportive environment. Travel within the community and to other locations is often required.
