Role Description
This is a full-time on-site role for a Physical Therapist located in Fremont, also PT position open for Los gatos and San Jose locations in CA . The Physical Therapist will be responsible for assessing patients’ needs, developing and implementing personalized treatment plans, monitoring progress, providing therapy for rehabilitation, and guiding patients in preventative care and home treatment plans. The role involves collaborating with a team to ensure optimal patient outcomes and delivering high-quality care.
Qualifications
- Expertise in Physical Therapy and Rehabilitation
- Ability to provide effective Home Care guidance to patients
- Excellent communication and interpersonal skills for interacting with patients and team members
- Relevant licensure and certification to practice as a Physical Therapist
