Responsibilities
What You Will Do:
- Work collaboratively with physicians, nurse practitioners, nurses, therapists, social workers, and caregivers to deliver comprehensive and coordinated care to our patients in their homes – conduct patient home visits
- Conduct evaluations and develop occupational therapy treatment plan for our patients
- Deliver treatment/care in accordance with care plan
- Prepare and maintain accurate and complete clinical and progress notes on a timely basis
Qualifications
Qualifications:
- Active Occupational Therapist state license
- Valid drivers’ license
- Two or more years of applicable occupational therapist experience
