Role Description The Registered Nurse will provide remote, contract-based nursing services that support clients accessing IMANI COMMUNITY OUTREACH CENTER’s programs. Day-to-day responsibilities include conducting virtual health assessments, triaging client concerns, and providing health education and counseling through telehealth platforms and phone consultations. The nurse will coordinate care plans, document clinical encounters in accordance with regulatory and organizational standards, and collaborate with other healthcare and community service providers to ensure continuity of care. The role also involves monitoring client progress, offering guidance on medication adherence and symptom management, and supporting referrals to appropriate medical or social services. This is a contract, remote role that requires reliable technology and adherence to privacy and confidentiality standards.
Qualifications
- Valid Registered Nurse (RN) license in good standing in the applicable state(s), with eligibility for multistate practice if needed.
- Experience in telehealth, community health, case management, or ambulatory care, preferably serving diverse or underserved populations.
- Strong clinical assessment and critical-thinking abilities, including triage, care planning, and evidence-based decision-making.
- Proficiency with electronic health records (EHR), telehealth platforms, and basic computer applications for documentation and communication.
- Excellent written and verbal communication skills, with the ability to provide clear, empathetic health education remotely.
- Demonstrated ability to work independently in a remote setting, manage time effectively, and maintain professional boundaries.
- Knowledge of HIPAA and other relevant privacy, safety, and regulatory requirements, and commitment to ethical practice.
- Associate or Bachelor’s degree in Nursing; additional certifications in community health, case management, or telehealth are a plus.
- Previous experience collaborating with multidisciplinary teams and community organizations is beneficial.
