Qualifications
- Experience with Discharge Planning and Case Management processes
- Proficiency in Utilization Management practices
- Strong foundation in Nursing and Medicine
- Excellent clinical decision-making, critical thinking, and organizational skills
- Effective communication and interpersonal skills to collaborate with diverse healthcare teams
- Active and unrestricted Registered Nurse (RN) license
- Prior experience in a case management role within healthcare is a plus
