Purpose of Position- Coordinates inclusive health care and community-based services, home visits, assessments and caregiver education for an assigned caseload. Independently develops, implements, and evaluates adequate care plans in a community setting, while working with other health care providers and agencies to integrate care. Provides quality leadership and oversight to caregivers providing direct care in the community. Works in close association with all members of Imagine! Services to ensure the delivery of high-quality nursing services.
Essential Duties/Responsibilities
- Responsible for nursing case management for assigned case load.
- Attends and participates in Person Centered Support Plan meetings for assigned case load. Attends Member Identified Team (MIT) meetings as needed and revises care plans as necessary.
- Writes individualized care protocols and educates caregivers on the implementation of the protocols.
- Monitors specialized care as indicated by State rules and regulations.
- Completes home visits as assigned and as needed.
- Provides direct care as needed; assigns and/or delegates nursing tasks within the scope of practice per Nurse Practice Act.
- Completes home visits in compliance with regulations and as assigned.
- Responsible for specific healthcare training and monitoring of caregivers.
- Provides QMAP training including MRB and CPR/1st Aid practicum.
- Assesses and monitors MRB use per state rules and regulations.
- Provides infection control training and education to caregivers.
- Supports infection control and mitigation efforts in residential settings.
- Reports errors/incidents to Management as needed.
- Collects, monitors, reports and records objective and subjective data.
- Contributes to assessment of health status.
- Observes conditions or change in conditions.
- Collects data and reports signs and symptoms of deviations from normal health status to the Manager of Nursing Services and/or client’s physician.
- Assists in formulating lists of needs/problems.
- Monitors all medical appointments and follows-up, processes and signs off on physician orders for clients on caseload.
- Professionally and positively represents Imagine! as a member of Nursing Services.
- Maintains respectful and productive relationships with other healthcare professionals, local and State agencies, committees, taskforces, commissions, teams, or other bodies which monitor or otherwise affect the operations of Nursing Services. This includes the Colorado Association of Nurses for the Developmentally Disabled (CANDD) as well as the Developmental Disabilities Nurses Associate (DDNA).
- Maintains respectful and productive relationships with people in services, their families and care providers, guardians, and Imagine! employees.
- Administers TB skin test and supports respiratory protection program.
- Accurately transcribes medication orders as prescribed.
- Writes informed consent for psychotropic medications.
- Reviews Quality Assurance for assigned case load and communicates with the appropriate team members.
- Attends team meetings as needed.
- Responds to client concerns and requests.
- Ensures Nursing Services are performed in compliance with all applicable Federal, State, County and Municipal statutes, regulations, rules and policies on a daily basis.
- This position has on-call responsibilities which will include alternating nights, weekends, and holidays.
- This position requires regular use of a personal vehicle with liability insurance that meets Imagine! standards.
- Responsible for the integrity and accuracy of data in the electronic health records and all other systems used in support of this program.
