Role Description
Families facing pediatric illness are navigating one of the most difficult chapters of their lives. The Case Manager is the person who guides them through it all - helping them think clearly in moments of crisis, connecting them to resources they didn’t know existed, and ensuring they never feel alone in the process. This role is deeply relational. This is not a traditional desk job. One day might involve sitting with a parent who needs guidance navigating a new diagnosis; another might involve coordinating services, meeting with community partners, or helping a family access a critical resource. The Case Manager will serve as the primary support professional for approximately 50 families, ensuring each family receives thoughtful, consistent, and personalized support. This is an opportunity to do meaningful work while helping build a stronger support network for families across the Washington, D.C. region.
This is a full-time hybrid role for a Case Manager based in the Washington DC area, with the flexibility to work from home on certain days. The Case Manager will work closely with families to assess their needs, develop personalized plans, coordinate resources, and provide continuous support. Responsibilities include facilitating communication between healthcare providers and families, connecting families to community and financial resources, and ensuring that care plans are executed effectively. The role also involves maintaining accurate records and providing emotional support to families during challenging times.
Qualifications
- A healthcare-related degree (such as social work, human services, public health, or healthcare navigation) is preferred but not required. We are looking for someone who is compassionate, resourceful, hardworking, and proactive.
- Excellent interpersonal, communication, and active listening skills
- Strong organizational and time management skills with attention to detail
- Familiarity with healthcare systems, community resources, and support networks is a plus
