Job Summary
We are seeking a Life Coach to assist with discharge planning, facilitate adjustment to nursing home placement, and maintain appropriate records.
Key Responsibilities:
- Maintain effective communication with residents, families, and interdisciplinary staff.
- Schedule and facilitate quarterly care plan reviews.
- Complete MDS as assigned and maintain resident and family contact.
- Assist with completing social histories and recommending referrals to community agencies.
- Compile information about community health and social services available.
- Accept and investigate grievances and complaints in a timely manner.
Requirements:
- Bachelor's degree in Social Work or equivalent.
- Minnesota Social Work License.

- Commitment to quality programs and services for seniors.
- Excellent communication and human relation skills.
- Ability to maintain confidentiality and exercise independent judgment.
