Duties
- Assist client with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Patient requires regular wound care for bed sores on various parts of the body.
- Administer medications accurately following care plans and HIPAA regulations to ensure patient privacy and safety
- Support clients with meal preparation, cooking nutritious meals tailored to dietary needs, and maintaining a clean living environment
- Utilize specialized equipment such as Hoyer lifts or wheelchairs to assist with heavy lifting and transfers safely
- Observe and document patient conditions, behaviors, and progress for care plans and communication with healthcare teams
- Provide companionship and emotional support, especially for individuals with dementia, Alzheimer's, or developmental disabilities
Requirements
- Proven experience in home & community care, assisted living, nursing homes, or group homes setting preferred
- Knowledge of patient observation techniques and ability to monitor vital signs accurately
- Experience working with people with disabilities, autism, dementia, or memory care needs is highly desirable
- Ability to perform heavy lifting safely and operate medical equipment like Hoyer lifts or mobility aids
- Strong communication skills for working effectively with clients, families, and healthcare professionals
- Valid driver’s license/reliable transportation are preferred for community-based support roles
