Responsibilities
- Perform nursing assessments and provide skilled hospice care in the home setting.
- Collaborate with patients, families, physicians, and interdisciplinary team members to develop comprehensive plans of care.
- Monitor patient conditions and communicate changes to the care team
- Provide education, support, and guidance to patients and caregivers.
- Promote comfort-focused care and symptom management.
- Ensure timely and accurate clinical documentation.
- A current license to practice as a Registered Nurse in the State of New York
- Associates Degree in Nursing, BSN preferred
- 1 year experience as an RN required-preferably in Hospice/Home Care or acute care.
- Valid Driver's License and Reliable vehicle
- Strong communication, organizational, and clinical assessment skills.
- Previous home care, hospice or palliative care experience preferred but not required
