Requirements
Requirements for working as a Community Health Worker
- High School Diploma or equivalent.
- Bachelor’s degree in a human services field, preferred but not required.
- Any combination of experience with health, education, and social services for a minimum of 1 year.
- Basic understanding of health conditions, such as HIV/AIDS, Diabetes, COPD, Asthma, CHF, CVD
- Bilingual - fluent Spanish speaking.
- Communicating to clients the purposes of the program and showing them, the impact it may have on them.
- Conducting intake interviews, enrolling and or referring individuals into community programs.
- Assisting health care providers involved in the clients care, to ensure clients understanding of care plans.
- Supporting the development, implantation, and execution of care plans.
- Monthly reporting, maintaining electronic client files, including updated release of information, assessments, and other medical documents.
- Travels extensively to client homes, community locations, agencies, and other outreach destinations.
- Day Shift Monday – Friday. No weekends or Holidays.
