We are looking for skilled MDS Care Plan Coordinator to become a part of our facility's compassionate and hospitable care-giving team. **We are a high volume, skilled nursing acute care facility where nursing assessment and review is crucial to our facility.
Hours: Monday-Friday 8am-5pm
