We are looking for skilled MDS Care Plan Coordinator to become a part of our facility's compassionate and hospitable care-giving team.
Hours: Monday-Friday 8am-5pm

Requirements
We are looking for an experienced LPN/RN with at least 3 - 5 years of MDS Coordinator experience. We are a high volume, skilled nursing acute care facility where nursing assessment and review is crucial to our facility.
Salary : $70,000 - $80,000
