We are seeking a motivated, compassionate, and detail-oriented Targeted Case Manager to join our care management team. This role supports individuals by coordinating services, connecting them to community resources, and ensuring care is delivered in full compliance with state and federal requirements. Key Responsibilities:
- Provide targeted case management services in accordance with state and federal regulations
- Conduct required quarterly in-person visits
- Assist individuals with applications and access to community-based services and supports
- Coordinate services across providers, agencies, and community resources
- Complete assessments, service plans, monitoring, and follow-up activities
- Maintain accurate, timely, and compliant documentation
- Advocate for individuals and support continuity and quality of care
- Maintain confidentiality and comply with HIPAA and all regulatory standards
Minimum Qualifications (one of the following):
- At least two years of experience in the delivery of social services, including applications and coordination of services
- OR
- At least two years of experience working as a case manager in a healthcare or social services setting
Preferred Qualifications:
- Strong knowledge of community resources, social service programs, and support agencies
- Familiarity with Medicaid programs and documentation requirements
- Strong written and verbal communication skills
- Highly organized with excellent time-management skills
- Comfortable working independently and managing a caseload
- Able to follow structured workflows and strict compliance standards
